  THE ROLE OF ERS (ENDORECTAL SONOGRAPHY) IN THE CLINICAL STAGING OF RECTUM ADENOCARCINOMA

  Background: To evaluate the significance of perirectal lymph nodes, detected by endorectal sonography (ERS) in patients treated of rectal adenocarcinoma.
Methods: From 1980 to 1993, 125 patients (10 N1) were treated by radiation therapy alone (contact therapy, EBRT ± Iridium 192 implants) in radiation department of H(tm)pital Lyon Sud in Lyon and 158 patients (36 N1) were treated by preoperative EBRT.
Results: For patients treated with contact therapy alone, only 3 perirectal lymph node relapses were observed. No loco regional relapse for patients treated by contact and EBRT. Out of 158 patients treated by preoperative irradiation, 5-year survival was 90% for uNO vs. 50% for uN1 : p = 0.04.
Conclusion: ERS is valuable for staging of rectal adenocarcinoma. For uN1 patients, contact x-ray alone is not recommended. For inoperable patients a combination of EBRT and a local boost with contact x-ray therapy and/or Iridium 192 is beneficial and can control uN1 patients.Preoperative radiation therapy is another alternative to control uN1 patients with results comparable to postoperative radiation with chemotherapy.
