POSTOPERATIVE BRACHYTHERAPY OF CERVIX UTERI CARCINOMA: MDR vs. HDR (THE ANALYSIS OF COMPLICATIONS ON RECTUM AND URINARY BLADDER)

Background: Previous surgical interevention in patients with cervix uteri carcinoma can be connected with a higher rate (5 to 10%) of complications occurrence after radiotherapy. Apart from this clinical problem the reason for complication development can be also found in Dose-Fractionation-Time relation. The aim of this study was to find out the most advantageous fractionation schedule, MDR or HDR, not only in terms of good local control of the disease, but also for the purpose of minimizing the occurrence of complications on rectum and urinary bladder. Methods: Two groups of thirty patients in each were analyzed. The first group consisted of patients treated with postoperative intracavitary brachytherapy by SELECTRON Cs-137 (MDR) in two applications of 2125 cGy per fraction, and second of patients irradiated by MICROSELECTRON Ir-192 (HDR) in five applications - one application of 700 cGy per week. Both groups were treated with external beam radiation by means of 15x energy photons and under same conditions. Results: The patients who were in MDRD radiation regimen developed complications in case of 50% and on urinary bladder in case of 53%. In case of the patients who were in HDR regimen the percentage of rectum complications was significantly lower - 23.3% and on urinary bladder 26.6%. Difference in the frequency of complication occurrence in these two groups was statistically proven but with 11.9% risk of error. Conclusion: Having in mind the obtained results and the occurrence of complications we can say that HDR radiation regimen is a method of choice, i.e. dose intensity and fractionation, in case of postoperative brachytherapy of cervix uteri carcinoma. |
